Death Certificates

In Person Request

You may request a certified death certificate from a local registrar at a county office or from the funeral home. If you request the certificate in person, you will be asked to complete and sign an application (PDF) Opens a New Window.  stating that you have tangible interest in the certificate. You will be asked to show identification.

If you do not have a tangible interest you may request a non-certified death certificate (PDF) Opens a New Window.  from our office as well. The fee is $13.

By Mail

To apply for a death certificate by mail, please fill out the application (PDF) Opens a New Window.  and have it notarized, enclose check or money order payable to Crow Wing County Land Services for $13 for a certified copy of the death certificate ($6 for each additional copy of the same certificate); mail to our office, and we'll be happy to fulfill your request.
